Carved in alabaster and coloured marble.
The inscription reads:

"Here lieth buried Mary Davies the wiffe of Richard Davies (the sonne of John Davies the elder of Middleton in the County of Salop Esq.) Esq., of the Body Exterordinary unto his Magisty. Shee was the eldest daughter and heire of Richard Cocke of this parishe Esq. the sonne of John Cocke, Esq. by Elizabeth his wiffe, one of the daughters of the Right Honerable Thomas Lord Wentworth Lord Chamberlyn to King Edward the 6. Shee departed this liffe without issue borne one the 2 day of September Anno 1623."

The coat of arms above the tablet are of John Cocke of Prittlewell, granted in 1587.
